*The Google Form is the first step in the registration process. You will also be required to complete the CCSD Athletic Registration in ParentVUE under the “Athletic Registration” tab. As of 3/17/25, the Athletic Registration forms have not been updated in ParentVUE. We will notify all players/parents who have completed the Google Form when the 2025-26 forms are available.

*All NEW players trying out for the Fall 2025 softball season are required to have a sports physical DATED ON OR AFTER 4/1/25 submitted before tryouts (through ParentVUE). The specific forms to be completed and submitted BEFORE tryouts are accessible in ParentVUE. RETURNING players’ physicals from last year will cover them through tryouts, but they will need a new physical dated after 4/1/25 before summer workouts begin.
